ABSTRACT

Objective: To estimate the diagnostic value of fibronectin type Ⅲ-domain containing protein 5 (FNDC5) in subclinical diabetic cardiomyopathy. Methods: A total of 94 patients with type 2 diabetes (T2DM), who were hospitalized from April 2018 to June 2019 in the Third Affiliated Hospital of Soochow University, were enrolled in this study. Patients were divided into T2DM with cardiac dysfunction (subclinical DCM) group (n=47) and T2DM without cardiac dysfunction (non-DCM) group (n=47) according to echocardiography and gated myocardial perfusion imaging results. Basic clinical data and serum FNDC5 level were compared between the two groups. Logistic regression analysis was used to establish predicting models and the diagnostic efficiency of established models was compared by ROC curve analysis. Results: Compared to non-DCM group, patients in subclinical DCM group were older, with longer duration of diabetes, and had higher levels of glycosylated hemoglobin A1c (HbA1c), total cholesterol (TC), triglyceride (TG) and low-density lipoprotein cholesterol (LDL-C) (all P<0.05). Serum FNDC5 level was significantly lower in subclinical DCM group than in non-DCM group (P<0.001). FNDC5 level was positively correlated with ventricular septal e'(r=0.451,P=0.005), mitral valve e'(r=0.291,P<0.001), the ratio of peak early diastolic trans-mitral flow velocity (E) to peak late diastolic trans-mitral flow velocity (A)(r=0.490,P=0.002), while negatively correlated with A(r=-0.399,P<0.001), the average ratio of E/e'(r=-0.490,P<0.001), tricuspid regurgitation velocity(r=-0.567,P<0.001), left atrial volume index(r=-0.491,P<0.001). Univariate ROC analysis showed that the diagnostic efficacy of FNDC5(AUC=0.940,95%CI 0.897-0.982)was superior to age(AUC=0.639,95%CI 0.523-0.752), diabetic duration(AUC=0.663,95%CI 0.555-0.772), HbA1c(AUC=0.740,95%CI 0.638-0.839), TG(AUC=0.661,95%CI 0.547-0.776), TC(AUC=0.675,95%CI 0.563-0.788)and LDL-C(AUC=0.644,95%CI 0.532-0.756). Model 1 was established with subclinical DCM as dependent variable, age, diabetic duration, TG, TC, LDL-C and HbA1c as independent variables. Model 2 was established by adding FNDC5 as independent variable on the basis of model 1. Diagnostic efficacy for subclinical DCM was compared between the two models by ROC analysis. The diagnostic efficiency was better with model 2 (AUC=0.980) than with model 1 (AUC=0.879, P<0.001). When sensitivity was set at 0.617, the specificity of model 2 was higher than that of model 1(0.979 vs. 0.936). When sensitivity was set at 0.532, the sensitivity of model 2 was higher than that of model 1 (1.000 vs. 0.915). Conclusions: Our findings suggest that serum FNDC5 could be used as a novel biomarker for the diagnosis of subclinical DCM.

ABSTRACT

Objective:To observe the clinical efficacy of Ditantang on phlegm turbid-blocked obstruction type tetan-burst twins in children with epilepsy and its effect on immunoglobulin, Peripheral blood helper T cells 17(Th17), interleukin-6 (IL-6) and interleukin-17A(IL-17A)in children with epilepsy. Method:The random distribution sequences of 100 children generated by the random number table method were placed in sealed envelopes, and the subjects were divided into the corresponding experimental group and treatment group. Control group was treated with sodium valproate oral solution, and treatment group was also treated with Ditantang in addition to the therapy of control group. The total course of treatment was 3 months. The changes of immunoglobulin M(IgM), immunoglobulin A(IgA),immunoglobulin G(IgG), peripheral blood Th17 cells,IL-6,IL-17A, C-reactive protein (hs-CRP), blood homocysteine (Hcy) and the adverse reactions were detected before and after treatment in two groups. The changes in the number of seizures, duration of seizures and EEG before treatment and 3 months after treatment were compared between the two groups. Result:Three months after the treatment of phlegm-blocking obstruction type tetan-burst twin epilepsy with Ditantang combined with sodium valproate, the results showed that the total effective rate of treatment group was superior to that of control group, with statistically significant differences (P<0.05). After treatment, treatment group was superior to control group in the reduction of epilepsy score (P<0.05). Compared with before treatment, the levels of IgG,IgA and IgM in treatment group were significantly increased (P<0.05), while the levels of Th17,IL-6,IL-17A,hs-CRP and Hcy in peripheral blood were significantly decreased. Compared with control group, the levels of Th17,IL-6,IL-17A,hs-CRP and Hcy in peripheral blood decreased significantly after treatment (P<0.05). The incidence of adverse reactions in control group was significantly higher than that in treatment group (P<0.05). Conclusion:Ditantang is effective and safe in treating phlegm turbidness and obstruction type epilepsy (tetan-episode twin), and can improve the clinical symptoms of children and electroencephalogram. Besides, it can improve the level of immunoglobulin in children, significantly reduce the levels of Th17,IL-6 and IL-17A in their peripheral blood, regulate the immune function of the body, and reduce the incidence of adverse reactions.

ABSTRACT

<p><b>OBJECTIVE</b>To study the effect of vitamin D (VitD) deficiency on cardiac autonomic nerve function in obese pre-school children.</p><p><b>METHODS</b>A total of 242 pre-school children with simple obesity were enrolled, and according to the serum 25-(OH) VitD level, they were divided into VitD deficiency group (76 children), VitD insufficiency group (83 children), and VitD sufficiency group (83 children). The three groups were compared in terms of deceleration capacity (DC) of heart rate, acceleration capacity (AC) of heart rate, and heart rate variability (HRV). The correlations of VitD level with DC, AC, and HRV were analyzed for the VitD insufficiency and VitD deficiency groups.</p><p><b>RESULTS</b>The VitD deficiency group had the lowest DC, root mean square of successive differences between adjacent RR intervals (RMSSD), and low-frequency power (LF) and the highest AC (P<0.05). The VitD insufficiency group had significantly lower DC, RMSSD, and LF and significantly higher AC compared with the VitD sufficiency group (P<0.05). The VitD deficiency group had significantly lower standard deviation of normal-to-normal RR intervals (SDNN) and high-frequency power (HF) than the VitD sufficiency group (P<0.05). In the VitD deficiency group, VitD level was positively correlated with DC, SDNN, standard deviation of average normal-to-normal RR intervals, RMSSD , LF, and HF and was negatively correlated with AC (P<0.05). In the VitD insufficiency group, VitD level was negatively correlated with AC (P<0.05).</p><p><b>CONCLUSIONS</b>Obese pre-school children with VitD insufficiency or deficiency have cardiac autonomic dysfunction, and cardiac vagal tone decreases with the reduction in VitD level.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To analyze the deceleration capacity (DC) of heart rate, acceleration capacity (AC) of heart rate, and heat rate variability (HRV) in obese school-age children, and to observe the correlations of BMI with DC, AC, and HRV in these children.</p><p><b>METHODS</b>A total of 108 obese school-age children were selected, including 75 cases of ortholiposis and 33 cases of dyslipidemia. A total of 103 healthy school-age children were selected as control group. All the subjects underwent 24-hour ambulatory electrocardiography. The comparisons of DC, AC, and HRV were made between the obese and control groups, as well as between children with ortholiposis and dyslipidemia in the obese group. The correlations of BMI with DC, AC, and HRV were analyzed in the obese group.</p><p><b>RESULTS</b>The obese group showed lower DC, standard deviation of normal-to-normal R-R intervals (SDNN), standard deviation of the average normal-to-normal intervals (SDANN), root mean square of successive differences (RMSSD), low-frequency power (LF), and high-frequency power (HF) than the control group. The AC of the obese group was significantly higher than that of the control group (P<0.05). In the obese group, children with dyslipidemia had significantly lower DC, SDNN, SDANN, RMSSD, LF, and HF, but significantly higher AC and BMI, as compared with those with ortholiposis (P<0.01). In the obese group, BMI was negatively correlated with DC, SDNN, SDANN, RMSSD, and HF (P<0.05), but positively correlated with AC (P<0.05).</p><p><b>CONCLUSIONS</b>Obese school-age children have impaired autonomic nerve function, presenting with reduced vagal tone, which is particularly prominent in those with dyslipidemia. The more obese the children, the lower the vagal tone, which may increase the risks of cardiovascular diseases.</p>

ABSTRACT

Objective To investigate the life quality of elderly patients who have received percutaneous coronary artery stent implantation,and to analyze the influence factors.Methods By using SF-12 scale and general information questionnaire,the investigation of the quality of life was conducted in 91elderly patients who had received percutaneous coronary artery stent implantation.Results The total score of the life quality of 91 elderly patients after receiving percutaneous coronary artery stent implantation was (68.06±17.72) points.The factors influencing the quality of life included age (statistic value=4.438,P<0.05),number of interventional therapy (statistic value=2.916,P<0.05),number of involved coronary artery vessels (statistic value=4.359,P<0.05),angina after operation (statistic value=-2.343,P<0.05),postoperative chest tightness (statistic value=-2.222,P<0.05) and smoking (statistic value=3.013,P<0.05).Conclusion The quality of life of the 91 elderly patients who have received percutaneous coronary artery stent implantation is at a moderate level,and it is influenced by many factors.Nursing staff should pay attention to the elderly patients after percutaneous coronary intervention and strengthen the health education guidance so as to provide theoretical basis for improving the quality of life of elderly patients.

ABSTRACT

Objective: To assess the role of Th9 and Th17 cells in malignant ascites (MA). Methods: MA from 30 hepatic carcinoma patients and benign ascites from 30 cirrhotic patients were collected. Corresponding peripheral blood samples from these hepatic carcinoma and cirrhotic patients as well as 30 healthy subjects were collected. The frequency of Th9 and Th17 cells was tested by flow cytometry. Serum levels of interleukin (IL)-9 and IL-17 were examined by ELISA. Results: The observed frequency of Th9 and Th17 cells, and the IL-9 and IL-17 serum levels were significantly higher in MA patients than those in cirrhotic patients and healthy control samples ( P<. 0.05). Moreover, the Th9 cells demonstrated positive correlation with Th17 cells as well as IL-9 in MA patients; however, this positive correlation was not observed in the cirrhotic patients or healthy control samples. The frequency of Th9 and Th17 cells was distinctly higher in MA patients presenting with stage III or IV malignancy and with lymph node or distant metastasis than those in patients in stage I or II and without distant metastasis ( P<. 0.05). Conclusions: The increased frequency of Th9 and Th17 cells in MA patients suggests that these two T cell subsets play a synergistic role in MA pathogenesis. This study also demonstrated that Th9 and Th17 cells may perform their biological functions in conjunction with IL-9 production.

ABSTRACT

This study was aimed to explore the effects of expressing eukaryotic elongation factor 1A1 (eEF1A1) on proliferation and apoptosis in human acute T lymphocytic leukemia (T-ALL) cell line Jurkat with knocked down eEF1A1 gene and its mechanisms. eEF1A1-expressing lentivirus (LV) was constructed and used to transfect the Jurkat cells with knocked down eEF1A1 gene. Then, the expressions of eEF1A1 mRNA and protein were detected by real time PCR(RT-PCR) and Western blot respectively.Cell proliferation, apoptosis and cycle were detected by MTT method, Annexin V-APC labeling and DNA ploidy analysis respectively. The related protein expressions of phosphatidylinositol-3-kinase (PI3K)/serine/threonine kinase (Akt) signaling pathway were detected by Western blot. The results indicated that eEF1A1 mRNA and protein expressions of Jurkat cells with knocked down eEF1A1 gene were re-established by constructing eEF1A1-expression LV. Compared with negative control group (transfected with negative control LV and eEF1A1-shRNA LV), cell proliferation in eEF1A1 expression group was significantly enhanced, cell apoptosis was remarkably inhibited, percentage of cells in G0/G1 phase was significantly reduced alone with increased percentage of cells in S and G2/M phase, and the expression levels of p-Akt (Ser 473), nuclear factor kappa B (NF-κB), p-NF-κB (Ser 468), mammalian target of rapamycin (mTOR) and p-mTOR (Ser 2448) protein significantly increased. It is concluded that eEF1A1 may have a carcinogenic effect in T-ALL cells. eEF1A1 expression has noticeable effects on the proliferation enhancement and apoptosis inhibition of Jurkat cells, which may be mediated by the up-regulation of PI3K/Akt/NF-κB and PI3K/Akt/ mTOR signaling pathway.

ABSTRACT

This study was purposed to investigate the effect of knocking down eukaryotic elongation factor 1A1 (eEF1A1) gene on the proliferation and apoptosis in human acute T lymphocytic leukemia (T-ALL) cell line Jurkat and explore its mechanism. The eEF1A1 mRNA and protein expressions of Jurkat cells and 3 healthy adult peripheral blood mononuclear cells (PBMNC) were detected by real time PCR and Western blot, respectively. eEF1A1-shRNA lentivirus was constructed through molecular biological method, and was used to transfect Jurkat cells. Then, cell eEF1A1 mRNA and protein expressions were detected by real time PCR and Western blot, respectively. Cell proliferation, apoptosis and cycle were detected by MTT method, Annexin V-APC labeling and DNA ploidy analysis, respectively. Cell-related protein expressions of phosphatidylinositol-3-kinase (PI3K)/serine/threonine kinase (Akt) signaling pathway were detected by Western blot. The results showed that eEF1A1 mRNA and protein expression levels of Jurkat cells were significantly higher than that of healthy adult PBMNC, respectively (P < 0.01, P < 0.05). eEF1A1 mRNA and protein expressions of Jurkat cells were significantly knocked down by constructing eEF1A1-shRNA lentivirus. Compared to negative control group (transfected with negative control-shRNA lentivirus), cell proliferation in eEF1A1-shRNA group was significantly inhibited, cell apoptosis was remarkably induced, cell cycle was blocked in G(0)/G(1) phase, and the expression levels of p-Akt (Ser 473), nuclear factor kappa B (NF-κB), p-NF-κB (Ser 468), mammalian target of rapamycin (mTOR) and p-mTOR (Ser 2448) proteins were significantly reduced. It is concluded that eEF1A1 may be a putative oncoprotein in T-ALL cells. Knocking down eEF1A1 gene has noticeable effects on the proliferation inhibition and apoptosis induction of Jurkat cells, which may be mediated by the down-regulation of PI3K/Akt/NF-κB and PI3K/Akt/mTOR signaling pathway.

ABSTRACT

<p><b>OBJECTIVE</b>To evaluate the diagnostic value of multislice spiral CT (MSCT) pneumocolon and image reconstruction in colorectal carcinomas.</p><p><b>METHODS</b>206 patients with suspected colorectal cancer underwent whole abdominal fast volume scan using 16-slice spiral CT before they were insufflated with 1000-1500 ml air through the anus. Then the axis data were transferred to workstation for imaging by MPR, SSD, Raysum, MIP and CTVE, respectively. The results of MSCT diagnosis were compared with those of surgical pathology and from some other common adjunctive techniques, such as ultrasonography (US), colonoscopy (CC), and etc.</p><p><b>RESULTS</b>In this group, 192 cases were confirmed to be colorectal cancer by surgical pathology. 189 cases were identified by MSCT. The sensitivity, specificity and accuracy was 98.4%, 92.8% and 98.1%, respectively. The accuracy rate of general tumor classification was 98.4%. These data were compared with those by US and CC, Statistical analysis showed that the qualitative diagnosis of MSCT is more accurate than those of other two examination methods, indicating that it is most valuable clinical detection approach at present.</p><p><b>CONCLUSION</b>MSCT pneumocolon is a superior detecting method as it is safe, quick and accurate. Due to the accuracy in orienting and qualitative diagnosis by comprehensive application of 2D and 3D imaging techniques, it can clearly display the detailed information of tumor in terms of morphological characteristics, blood-supply resources and pericolonic lymph nodes involvement, which provides reliable basis for making rational surgical protocol.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To detect novel mutations in the fibrillin 1 (FBN1) and transforming growth factor beta receptor type II (TGFBR2) genes by screening the genes from 14 patients with Marfan syndrome.</p><p><b>METHODS</b>Denaturing high performance liquid chromatography (DHPLC) was introduced to screen for FBN1 and TGFBR2 mutations exon-by-exon. The DNA amplification fragments which DHPLC elution profiles showed different from the corresponding normal elution profile were sequenced to identify the positions and types of mutations. Restriction fragment length polymorphism (RFLP) was employed to further prove the mutations when needed.</p><p><b>RESULTS</b>Two gene mutations of the FBN1 were found in the patients with Marfan syndrome. They were a novel substitutional mutation (Intron29 +4A > T) of FBN1 and a recurrent nonsense mutation (8080C >T) of FBN1.</p><p><b>CONCLUSION</b>Intron29 +4A > T and 8080C > T of FBN1 are possibly the pathogenesis of the MFS patients.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To explore the clinical therapeutic effect of acupuncture on hypertension of phlegm-stasis blocking collateral type and the mechanism.</p><p><b>METHODS</b>Sixty cases of hypertension were randomly divided into a treatment group and a control group, 30 cases in each group. The treatment group were treated with acupuncture at Fengchi (GB 20), Quchi (LI 11), Neiguan (PC 6), Zusanli (ST 36), Fenglong (ST 40), Taicehong (LR 3), and oral administration of Captori, and the control group only with Captoril, for 4 therapeutic courses. The changes of blood pressure, clinical symptoms, hemorheologic parameters, C-reactive protein (CRP), TC, TG, HDL and LDL levels after treatment were observed.</p><p><b>RESULTS</b>In the treatment group, blood pressure significantly decreased, and contents of CRP, TC, TG and LDHD decrease, the hemorheologic parameters improved and HDL level increased. Both the cumulative scores of clinical symptoms and the therapeutic effect for each symptom were improved significantly, with very significant differences as compared with those before treatment and the control group (P < 0.01).</p><p><b>CONCLUSION</b>Acupuncture treatment has obvious effect of decreasing blood pressure, and reverses or delays the course of atherosclerosis, which mechanisms are possibly related with the function of protecting vascular endothelium.</p>

ABSTRACT

Objective To investigate the relationship between children history of asthma and family history of asthma.Methods Three thousand and five hundred outpatients of asthma children were selected as investigation subjects.Family history of asthma was investigated in the form of questionnaire,and analyzed them from whether or not having family history of asthma,the first and second degree relative having family history of asthma.Results There was no family history of asthma in 1 659 cases(47.4%) and while 1 841 cases had family history of asthma(52.6%).The children who had family history of asthma were slightly more than those who had no family history of asthma.Among the 1 841 cases with family history of asthma,the first degree relatives with family history of asthma were more than those second degree relatives with fa-mily history of asthma.The incidence rate of relatives with family history of asthma on mother′s side was 59.9%,and the incidence rate of the father′s relatives was 40.1%.The incidence rate of mother′s relatives was higher than that of father′s relatives(P

ABSTRACT

Objective To explore the occurrence of diaphragmatic muscle fatigue(DMF) in children with pneumonia and its relationship with respiratory failure.Methods One hundred and twenty-six cases of pneumonia(pneumonia group) and 60 healthy children(healthy control group) were selected,and 2K-V1 type intelligence electric impedance respirograph was used to measure diaphragmatic function of the children in both groups to find if the ribcage-abdomen motion was synchronic and to calculate M levels.The abdomen impedance values as X-axis were calculated,which were regarded,and the chest impedance values as Y-axis were calcaulated,which described two-dimensional chart of ribcage-abdomen motion curve and the ? angle which was between connection of maximum X value to the smallest X value and the X-axis were calculated.According to the changes of chest-abdominal curve in one-dimensional chart and the changes of M value and ? angle,the degree of the diaphragmatic muscle fatigue was judged.Results There were 98 cases of DMF in the 126 children with pneumonia,and 68 cases of which were type Ⅰ DMF,and 30 cases were type Ⅱ DMF.One-dimensional chart showed contradictory chest-abdominal respiratory motional curve,and two-dimensional chart moved in a clockwise direction.M value was(46.1?8.4)%,? angle was(136.7?12.0) degrees in children with type Ⅰ DMF;One-dimensional chart showed that the peak value of chest-abdominal respiratory motional curve dislocates,and two-dimensional chart moved in a counter-clockwise direction.M value was(17.2 ? 3.2)%,? angle was(48.2 ? 9.5) degrees in children with type Ⅱ DMF;the control group had M value(4.3 ? 1.0)%,? angle was(31.7 ? 5.2) degree.There were significant differences in M value and ? angle between type Ⅰ DMF children,type Ⅱ DMF children and healthy children(Pa
